First have your MD fax RX/Insurance card (if we are billing insurance), CDL and proof of vacination to (530) 626-3049.
As a courtesy we can bill Medicare; Blue Cross, Blue Shield PPO as providers but encourage self accountability for knowing your coverage so please call the # on the back of your card to confirm we are in your network.
Sorry No NO HMO's, Worker's Comp or Covered California/MediCal coverage at this time.
After your information has been recieved/reviewed we will call to schedule an appointment.
We do accept cash and you can leave a message by calling at 530-621-1149.
COVID Infection control in place:
Please call 3 minuntes before appointments from your car to be screened and have ALL of the paperwork completed before 1st appointment. No public restrooms.
